Support plans

Precinct Care
Support, not care: each young person has a support plan built around their pathway plan, and works towards living independently, recorded under the 2023 Regulations.
Since October 2023, Ofsted registers and inspects supported accommodation for looked-after 16 and 17 year olds and care leavers, under the Supported Accommodation (England) Regulations 2023. Inspection looks at four quality standards: leadership and management, support, accommodation, and protection.
Precinct Care changes its words, forms and rules for supported accommodation, so staff aren't filling in children's home forms that don't fit.
Where we are: Precinct Care is in final development, shaped with registered managers, Responsible Individuals and Reg 44 visitors. We are booking demonstrations now and taking a small number of pilot homes. The snippets on this page are from the working system, filled with made-up people and records.
Built around the local authority's pathway plan, with the young person's own goals.
Budgeting, cooking, managing a tenancy, registering with a GP, getting to college or work.
Reviews with the personal adviser tracked, with actions followed up.
Attendance, job search, volunteering and training recorded.
Evidence gathered against the four standards for Ofsted.
Profiles, episodes and return interviews, as for children's homes.
Each young person's file has an independence section, with the skills they are working on and the progress they have made. Keywork sessions can be linked to a skill, such as practising paying a bill, so the evidence builds up as they go.
Young people get their own app too, with their support plan in plain words. See the young person's app.
Precinct Care runs children's homes and supported accommodation side by side. Each service follows its own rules, and an area manager sees them together. See children's home software.
Book a 30-minute demonstration. We'll show you the screens your staff, managers and inspectors would use.
