Missing from care profile
Everything the police need if a young person goes missing, kept ready in advance in the Philomena Protocol style.
Free, editable templates for children's homes, written to current guidance. Download them as Word to adapt for your home, or as PDF to print. No sign-up needed.
We made these while building Precinct Care, the children's home recording system from QLine IT and Precinct Software, where the same forms are built in. You are free to copy and adapt them for your own homes.
They cover children's homes in England. Wales and Scotland have their own rules, which we cover on our Scotland and Wales page. Check any template against your own policies and your local police and safeguarding partnership's procedures.

For the independent person: who they spoke to, records inspected, last month's actions, their opinion and recommendations.
What to do when a young person comes back, the welfare conversation, and the follow-up afterwards.

A profile for each young person, kept up to date so the person on shift can give the police everything at once, at any time of day: description and photo, health and medication, phones and social media, places they may go, people they may be with, missing history, risks and who to tell.
Review it at least every three months, after every missing episode and whenever something changes. Many police forces have their own Philomena Protocol form: if yours does, use theirs and keep this as your working copy. Read more about missing from care in Precinct Care.

For the independent person's monthly visit under regulation 44 of the Children's Homes (England) Regulations 2015. It follows what the regulation asks for: who was spoken to and whether in private, the premises, the records inspected, progress on last month's recommendations, and an opinion on whether children are effectively safeguarded and whether the conduct of the home promotes their well-being.
The report goes to Ofsted by the end of the month after the visit. Read our guide to Reg 44 visits, or see Reg 44 visits in Precinct Care.

For the staff on shift when a young person returns after being missing: what to do straight away, signs of exploitation to look out for, prompts for the welfare conversation, and the follow-up, including chasing the independent return home interview and deciding whether a Reg 40 notification is needed.
This is the home's own conversation. It is not the independent return home interview, which the placing authority must offer within 72 hours.
In Precinct Care the missing profile is kept up to date from the young person's file, the Reg 44 visit pack is built from the month's records, and the return checklist opens on its own when a missing episode is closed. Read about Ofsted report cards, our buyer's guide, or book a demonstration.
